Registration for DAP tasks with UCL School of Pharmacy
This online registration form for DAPs should only be completed by practitioners registered with UCL, School of Pharmacy.
If you are a practitioner with one of the other Universities (eg Brighton, Medway, East Anglia, or Reading) please do not complete this form. You should contact your individual University academic leads about the application process.
Practitioners should only complete the on-line registration form when they know the DAP Task they wish to register for.
