Please follow the link below to access the online registration form for the March 2012 intake, registration will be open until 5pm, 16th March 2012. To complete the form you will need the following pieces of information:
- Select School of Pharmacy London as your academic centre.
- You will need to know who is the Educational Programme Director (Lead Trust Tutor) at your hospital and their email address.
- You will need to know the name and email of your assigned Educational Supervisor (Practice Tutor). If this information is not known at the time of registration it will be necessary to inform pg.admin@pharmacy.ac.uk at a later date.
- You will need to provide the name and address of your sponsor who will be invoiced for your fees. In addition, if your hospital requires a purchase order number then this will have to be specified. (Please contact your Educational Programme Director (Lead Trust Tutor) if you are unsure about this point)
